Application or Removal of Medicated Trans-dermal Patch

Goal:

  • To apply/remove medicated transdermal patch.

Supplies:

  • Medicated transdermal patch
  • Gloves
  • Washcloth

Schedule:

  • 1 __________ check if patch has been applied in the morning. If not applied, then do so.

Placement:

  • Rotating sites

Reportable Observations:

  • 1 Inability to complete task.
  • 2 Skin Irritation.
  • 3 Incorrect number of patches found on client’s body.
  • 4 Change in client condition.

Storage Space:

  • Cupboard beside or above fridge
  • Cupboard beside or above kitchen sink
  • Other: Kitchen table?

Application of a Medicated Patch

Procedure Rationale
Wash hands.To maintain cleanliness as per standard precautions.
Gather supplies and assemble on a clean, dry surface.To complete the task in an organized manner.
Check the box for correct client name and medication.To ensure correct medicated transdermal patch is applied as ordered.
Put gloves on.To maintain cleanliness as per standard precautions.
Check the label on the box for client correct name and medication. Remove one medicated patch from box,To ensure correct medicated transdermal patch is applied as ordered.
Remove old patch as needed. Fold old patch inward, adhesive to adhesive and discard in garbage. Wash and dry skin where patch has been.To prevent overdose of medication. To maintain clean and healthy skin. To prevent transfer of medication to caregiver.
Check the label on the box for correct client name and medication,To ensure correct medicated transdermal patch is applied as ordered.
Using an ink pen, write current date (today’s date on the medicated patch).To show, once the patch is applied, it is the correct day’s patch on the client.
Apply new patch, rotating sites. Do not apply to irritated or noticeably hair-covered skin.To reduce the incidence of skin irritation, To allow optimal skin contact.
Remove gloves and discard in garbage. Wash hands.To maintain in cleanliness as per standard precautions.
Sign flow sheet. Report and record any reportable observations.To maintain accurate records.
